Academic transcripts for both the traditional and adult learning programs are issued by written authorization of the individual to whom the record belongs, or by way of that individual filling out St. Gregory’s Official Transcript Request form.
To request a transcript in person, visit the St. Gregory’s Registrar’s Office. It is located on the first floor of Benedictine Hall on St. Gregory’s Shawnee campus. A photo I.D. is required at time of request.
To request a transcript by mail, the following must be included in the correspondence:
- Full Name (as was while enrolled at SGU)
- Social Security Number
- Date of birth
- Last date enrolled
- Current address and phone number
- Address where transcript is to be sent
- Student’s signature
Mail requests can also be made via the Official Transcript Request form.
Transcripts cost $10 each, and payment is required at time of request. Transcripts are not processed if the student has an outstanding financial obligation to St. Gregory’s or a balance due on his or her student account.
Requests for transcripts are not accepted over the phone, however, if you need further information, contact the St. Gregory’s Registrar office at (405) 878-5434.
